New Patient Information
First Visit
In preparation for your first visit to Orthopaedic Associates of Michigan, please fill out and bring the Financial Policy & Privacy Notice Acknowledgement form (111 KB, PDF), which was mailed to you when your appointment was made. You can also find copies of the Medicare Financial Policy & Privacy Notice Acknowledgement form (98 KB, PDF) on this site. Once you arrive, you will meet our medical staff and your physician.
Insurance Forms
The Hand & Upper Extremity Center Medical History Information (439 KB, PDF) form, the Hand & Upper Extremity Center Patient Information Sheet (317 KB, PDF) and if appropriate, the Priority Health Questionnaire (401 KB, PDF) require completion prior to your visit. Please download the appropriate form(s), fill them out, and bring the completed form(s) with you to your scheduled visit. This will help expedite the registration process.
Office Policies
Children under the age of 18 must have a parent with them during each office visit.
Payment is due at the time of service unless other arrangements are approved in advance. We will file a courtesy claim to all insurance carriers. For your convenience, we accept Visa, Mastercard and Discover. Should you have any questions regarding our fees or your insurance coverage, please call us in advance of your appointment. Please have your insurance information available at each visit.
